The Delta State Government has constituted an eight-member Administrative Panel of Inquiry to investigate the circumstances surrounding the death of Mr Adeda Ekwevbughe, a Deputy Director in the Ministry of Basic and Secondary Education, amid concerns over the incident at his workplace.
The state government announced the panel in a special statement signed by the Secretary to the State Government, Dr Kingsley Eze Emu, on September 25, 2026, saying the incident had provoked serious concerns among civil servants, organised labour and the government.

Delta Govt Announces Administrative Panel
According to the government announcement, the Police had already commenced an investigation into the incident before the government consulted with Organised Labour on the need for an administrative inquiry.
The eight-member panel is chaired by Mrs Charity Ehiemhen, Permanent Secretary, Office of the Head of Service.
Other members are Mr Mamuzo Ebere, SAN, Solicitor-General and Permanent Secretary, Ministry of Justice; Mr Lucky Nkemachor, Permanent Secretary, Office of the Secretary to the State Government; and Sir A.E. Oghoro, Executive Assistant to the Governor on Public Service.
The panel also includes Comrade Goodluck Ofobruku, Chairman of the Nigeria Labour Congress, Delta State; Comrade Chief Benson Efeotor, Chairman of the Joint Negotiating Council; Comrade Wilson Asekutu, Chairman of the Trade Union Congress; and Comrade Dennis Chuks Isichei, Secretary of the Joint Negotiating Council, who will serve as Member/Secretary.
Panel Given Two Weeks to Complete Investigation
The Delta State Government said the panel has two weeks to complete its assignment.
Its mandate is to investigate the circumstances surrounding Ekwevbughe’s death and enable the government to deal fairly and squarely with all issues arising from the incident within the scope of the law. The administrative inquiry will run alongside the police investigation into the matter.

Delta Govt Assures Civil Servants of Safety
The state government expressed condolences to the family of the deceased and assured civil servants of the safety of their work environment.
It also reiterated its commitment to upholding respect for the dignity and sanctity of life, and appealed for due process as the Police and Administrative Panel continue their respective investigations.
