The Round of 16 encounter saw both sides score from the penalty spot in a tense first half, with Morgan Rogers giving Chelsea the lead before Cody Gakpo equalised for the hosts.
Rogers converted in the 40th minute after Ryan Gravenberch fouled Moises Caicedo inside the penalty area.
However, Liverpool responded three minutes later when Chelsea defender Wesley Fofana handled the ball in the box, allowing Gakpo to level the score.
Neither side could find a winner during the remaining minutes, forcing the tie into a penalty shootout without extra time.
Fofana Miss Settles Shootout
Liverpool eventually prevailed after Chelsea’s Wesley Fofana missed the decisive spot-kick, firing his effort over the crossbar.
Chelsea goalkeeper Mike Penders had earlier saved Ryan Gravenberch’s attempt, while Liverpool goalkeeper Giorgi Mamardashvili denied Emmanuel Emegha to keep his side in contention.
Joe Gomez converted his penalty for Liverpool before Fofana’s miss settled the contest in favour of the home side.
The victory extended Liverpool’s remarkable record against Chelsea in penalty shootouts, marking their fifth consecutive triumph over the London club in such circumstances.
Both managers made several changes to their starting line-ups following their respective Premier League fixtures at the weekend.
Liverpool manager Andoni Iraola introduced Florian Wirtz and Bradley Barcola before the hour mark as his side sought a winning goal.
Chelsea also created opportunities, but neither team could break the deadlock.
Mamardashvili produced important saves to deny Ronald Araujo and Barcola, while Marco Palestra tested the Liverpool goalkeeper at the other end.

Pressure Mounts On Alonso
Chelsea manager Xabi Alonso, who returned to Anfield after previously winning the Champions League with Liverpool as a player in 2005, was left disappointed as his side exited the competition.
The defeat followed Chelsea’s 2-0 Premier League loss to Tottenham Hotspur, adding to the pressure on Alonso as he seeks to establish his team this season.
Liverpool, meanwhile, celebrated their fifth consecutive victory in all competitions, with the last three coming at home.
The Reds will now turn their attention to Sunday’s Premier League clash against Arsenal, with the opportunity to strengthen their position in the title race.
Liverpool will face Bournemouth in the Carabao Cup quarter-finals as they continue their pursuit of another major domestic trophy.
